C axis milling

if the X-offset is off, the slot will be curved.

With C-axis milling, to move a feature, you can not simply offset the X axis. For example, if you program a 1" square, and the offset X .25, you will not get a 1.25" square. The 4 sides will be curved because X/C values would need to be changed in the program.

If the control has the option to convert XY code, to XC motion (maybe G114), you could do a test cut to see if it is a problem with the machine setup or if the problem is with the code.
