For CNC work, we've used Erix flip-out tools in the past. Not too bad, but not 100% reliable, especially in the larger diameters (over 2"). No flip-out/fold-up = big crash.

 

We're experimenting now with a couple of solid tools designed by Vermont Indexable Tool. No moving parts, just one insert per tool. Using two different tools in combination, we've been able to back-spotface a 2.75 diameter in cast iron, going through a 1.188 drilled hole. Just getting started with them, so can't comment on how they'll be over the long haul, but the first holes have looked very good.
