I use the 5 Axis Flow in 4 Axis mode quite often.

 

Are you sure that you are selecting the proper axis to use to rotate the 4th around ???

 

And your tool/Cplane should be set to what you would use for normal 3 axis machining.

 

IE. vertical mill would be TOP and TOP

 

You should get a position move ( for your 5th axis) at the beginning of the program and then XYZ and your 4th after that.
