Groups? File > Merge adds new groups to your file. Use the Group Manager to delete any phantom groups in the file.

Machine Definition File > when editing your Machine Definition, it is possible you accidentally saved solids into your Machine Definition, and these solids are loaded (kind of invisibly) into your Mastercam File. Those solids could be hold-overs for a long time, if you have been updating an older Machine Definition File for many versions in Mastercam.
