Delete the second operation. On the first operation, add the chain(spline, pick front) Turn off the T/C Plane Switch. Go to the Rotary Axis button, pick Axis Substitution, substitute z Axis, put in the diameter (10.1875) Pick CCW and Unroll leave the unroll tolerance alone unless you need to change it.

This is just general information on slots in cylinders. In order to get parallel sides on slots, use ROLLDIE.DLL with the SLOT option, or use CURVE5AX. For either one, drive the centerline of the slot only. ROLLDIE can work from the flat. CURVE5AX must have the cylinder surface in order to find normals, so you can't work from the flat.
