How do tools get corrupted?

What I like is when you are in lathe and there is an error. It does not tell you what the problem is or show you the path to try and fix it. It just throws up an alarm so you can play "Needle In A Haystack".

When I hit ok it removes the tools. If I hit no it was fine and back plot ok, posts in camplete ok. 

 

One was an endmill which I changed the size and description, the other was a thread mill that I made this morning.
