You have to 'shorten' the lead out contour chain or else your tool will be trying to cut with the backside of the holder and give you lotsa problems.
In backplot, click on display tab,and on the backplot display 'general tab, make sure that simulate rotary axis is checked on.
Welcome to the forum.

I took out my lead in and out and it runs. The only time i had similar problems is when I had too small of an arc radius value in my lead in/lead out.
I use the FLATSRF.DLL
works like a charm
With the dim's you gave, you should end up with approx:
Big arc= 24.514 (chord length of 27.712)
Small arc= 22.553 (chord length of 25.495)
2 legs 1.961 long
This is all to the 'mean'of the mtl. (middle of the material thickness)

Your machine is prolly a 1984 or 1985.
Jus curious, does it use work offset or G92?
The guys in my area Mass. that know the fanuc/Hitachi's is Boston machine in Peabody mass.
We have some of these old Hitachi clunkers and it is very funny how just as the heat and humidity came, the machines started to act up!!
We have a system in place but it is more of a spreadsheet type.
Each machine has a 3 ring binder type notebook with a daily/monthly checklist that the guys just breeze thru in the morning.
Breeze thru meaning to just sign it off,not actually checking anything on the machine.
It only looks good on parer for ISO....
mastercam no.
but I bought this disc from cutdata for 500 beans a while ago and it gives you surf. footage, feed matl.removal rate, power, cut time ipt.....

Been there...
hit the offset button twice to go from 'workoffset to height/diam. offset'.
on the workoffset page, there will be 6 places you can store your G54-G59
on the other page, you set your height offsets.
Ther is no diam. offset page so use your higher offset numbers for diam. comp.
Ex. D51
We have a few of these machines with the 6M control
